Child Death Overview Panel

This report (available here) provides an outline of the new Child Death Review Arrangements to be implemented across Southend, Essex and Thurrock in response to the Children and Social Work Act 2017 and Working Together to Safeguarding Children 2018.

The legislation introduces new Child Death Review Partners, consisting of the Local Authority and Clinical Commissioning Groups (CCGs), to oversee all child deaths in their area. The Child Death Review Arrangements are to become operational from 29th September 2019.

Notification of a child death

Any agency becoming aware of:


a child death occurring in Essex, Thurrock or Southend


a death of a child normally resident in Essex, Thurrock or Southend occurring elsewhere

should make a notification to the SET Child Death Review Manager via their nominated Child Death Review lead.

The notification should be made on the Notification of Child Death form
